Recovery does not end within the walls of a clinic. It continues in the home, the workplace and the community. Elpida’s Community Rehabilitation program is designed to support clients as they transition from clinical care back into everyday life, so that progress made in treatment is sustained long-term.
This program focuses on helping clients rebuild social connections, re-establish family relationships and reintegrate into community and religious life following a period of mental health or neurological rehabilitation. Our team works closely with clients and their families to identify practical barriers to reintegration and to develop strategies that overcome them.
Community-based support may include home visits, family education sessions, peer support connections and coordination with local community structures. We recognize that every client’s community context is different, so our approach stays flexible enough to accommodate diverse family and social settings across Port Harcourt and beyond.
By bridging the gap between clinical treatment and everyday living, our Community Rehabilitation program helps reduce relapse, prevent isolation and restore clients’ sense of belonging, a vital part of any lasting recovery journey.
